I know someone that confuses both and I've been trying to help him. I've tried to help him and I can't figure out how. It's like he mixed D&D reality and nearly killed this guy over a chiken with a sword. Can someone help me??? And please no stupid or useless answers.

Remember that everyone's "reality" can be very different from our own. Some ppl cannot deal with the reality in this world and escape into their own reality, which can fully seem like fantasy to us. Try talking with him about this? See if he wants or needs help, and if so, suggest a therapist. This isn't something that you can 'fix' in him or for him. It needs to come from inside him. Being a danger to himself or others can land him in the mental hospital for a period of time. If he continues to be dangerous and there's little you can do, call the police who will get him to some well-needed help. Unfortunately, there's not much you can do to 'fix' him. You could get yourself hurt, or worse, killed. Let a professional deal with him. Best you can do is to report him, esp. if things worsen.

He definitely needs mental help. Anytime a person experiences a difference between reality and their perception of reality something has gone wrong. And if he has acted violently in the past, time is of the essence. Medications can help, but a professional is the only one who can really help your friend.

firstly are you a mental health professional?

I'm assuming not as you wouldn't be posing this question otherwise, so that said:
1. you can't take responsibility for someone that is ill, all you can do is support them.
2. they need proper medical help, contact their doctor, mental health, someone appropriate to take over this.
3.YOU will not cure them as you do not have the technical skill, psychological and pharmacological so stop 'deluding' yourself and putting your emotional life at risk.
4. read scripts of peoples lives (something like that) Eric Berne, and maybe you can help yourself to stop getting roped into a helper role.

And on personal note based on experience, be very, very careful!

If he is a danger to himself or society, then you should take steps to get him help. The person that he threatened could file charges against him. At that time, he can be evaluated by professionals. If he can not afford help, then the state would pay for the evaluations and the process to have him committed for professional care could take place.

I would definitely advise your friend to go see a psychiatrist to work out the issue. There are even options of going to see an art therapist or recreational therapist who can help him come to the realization b/t reality and fantasy. Hope that helps. With a psychotherapist.....or, he needs some kind of horrific experience that will make him snap out of it and realise that he's living in the REAL world whether he likes it or not.

I believe that this is a mental disorder and would require professional help, at least short-term.
