RPM means revolutions per minute. The instrument that measures this is called the tachometer. It is measuring how fast the engine is running. Since the ICE (internal combustion engine) is most efficient in a relatively small speed range, the tach helps you decide the best time to shift for the way you are driving. Usually for fuel efficiency, you would shift in the lower ranges of engine speed. If you are racing, you would shift at higher engine speed where the engine has more power. Also, most engines will have a "redline", meaning it should not be run at higher than that speed or you risk damaging your engine. On automatic cars, the transmission takes care of shifting at the right time for you. Your owner's manual may describe recommended shift points for normal driving.

RPM gauges indicate the revolutions per minute you engine is turning at the time you read the gauge. You can use the gauge for many purposes. One of the most common usage is to determine when to shift your manual transmission.

On semi's they are used to let you know when it is time to shift gears. On automatic cars it is for basically the same thing, if you are going uphill or pulling a heave load you may want to switch to a lower gear to save wear and tear on the motor.
