The ex stands for executive. An executive secretary works for those "higher up" in the company such as the president or vice president of a company, whereas a regular secretary would work for a low level manager or something similar. An executive secretary would need more experience, education and/or skills.

The last answer is correct, it stands for executive. You often have more responsiblities and act as a "gatekeeper" for the Exec. along with secretary duties.
