it is a term usually used when referring to working out. What happens is that while you are working out your muscles will become weaker from use and then tend to hurt because they stretch and are put against force (weights). YOur bodys natural reaction to that is to make your muscles thicker.
So when they say no pain no gain it means if you dont feel pain then your muscles were not damaged to the point wwere your body needs to rebuild them and you will not see the results that you want

You have to put in some effort to see improvement... Originated in athletic workouts... If you are lifting weights and stick with an easy weight, you're not going to improve. You have to increase the effort (weight) until it is difficult to achieve if you want to improve....

it means that things you really want are not just going to be handed to you. You have to commit to your goals and pay for them with sweat, blood and tears for it to be worthwhile in the long run
