They are the opposite ends of the river--the mouth is where the river empties into another body of water; the source is the headwaters, or the little streams that feed into the river. The outlet is the same as the mouth.

The source of a river is the starting point of a river where water first enters it. The mouth is the ending point of the river where it is meets a bigger body of water (normally where it joins the sea).

In the course of a river, there are many small outlets that branches out from the main river. This normally occurs at the lower end of the river, i.e. near the mouth.

The mouth of a river is where it enters its base level, such as a lake, pond, ocean, etc. The source of the river is where the water actually comes from. This can be a spring, runoff, melt or other sources of water.

The definition of an outlet of a river depends on specifically what you are referring to. For the purposes of this question, I assume that the answer you are looking for is that an outlet and mouth of a river are the same thing.

The source, derived from the French spring of a river, stream, etc. It is the point where the water springs out from the rocks and is considered as the start of a water course.
The mouth is the point where the river ends its course, it could be the sea or another water course. The mouth could be an estuary, a single way out, or a delta, D form, several ways out.

They are at opposite ends of the river. The source is where it begins (up in the hills probably) and the mouth is where it runs into the sea.
