Before planning and development for HR, the performance evaluation the year before should be assessed, so that one could determine whether the previous performance tool was effective.
This is one example of a performance tool for College Instructors.
Part I. Describe the innovation that you introduced in your class during the previous academic year. Indicate if such innovation was adopted by your program, college or the entire university. Also, describe briefly the impact of the innovations in terms of changes and improvement in the quality of education or more specifically in the teaching-learning process. Include also the patents or copyrights you have obtained during the year and describe briefly its value to your program, college or university.
Part II. Indicate personal and professional outputs that contributed to the improvement of the quality of your work. Items to be included should show proof of professional competence and effectiveness such as high evaluation/ratings from students, commendations received from the different stakeholders of the institution, promotions obtained as a teacher from supervisors and administrators, awards received from teaching, membership and participation in professional organisations, training programs attended, new knowledge or skills learned from continuing education and others.
Part III. Indicate here if you have submitted all college / unit requirements during the previous academic year. The following should be considered in this factor: submission of grades, attendance in meetings, and compliance to college assignments such as coaching, mentoring, proctoring and student-consultation responsibilities among others.
This is only an example. You can modify it to suit other professions.
