If you not 100% sure who's coming I always tell them to bring their gift and you pick the "order" once you get to the party. That way no one is left out and prior to starting the exchange you can have a little ceremony to put all the names in a hat and pull them out. In an group exchange the person with the highest number picks last.
A themed gift exchange is fun:
1. Favorite Drink - wine, whiskey, etc
2. Elephant Gift - last years worst
3. Regift Exchange
4. Food
5. Classic Toys of your generation - i'm a 70's-80's guy so someone would bring a simon, original stompers, etc,

To draw the names in the first place it is fun to write the name or initials of the people on the inside bottom of a paper cup, fill the cups with a colored beverage and set them on a serving tray. Each person selects a cup, drinks and buys a gift for whoever's name is written in their cup.
Don't tell who is buying for who.
When it is time for the gift exchange the person opening the gift has to guess who got the gift for them and why.

Names on bottoms of plates, if at work have a breakfast and put the names on the bottom of the paper plates and if for family, and everyone will be together, then put on bottom of chair, or plates if using paper. If not, you could write everyone's name on a napkin, or slip it into a cloth napkin, and simply set the table with them.
