Is it by the first word of the citation, the book title, or what?

The first word. This is usually the author's name, but if there is no author, it can be the article title, journal name, book title, etc. Depends on what citation style you used and what the source is. Make sure your in-text citations are by that first word as well (if it's MLA).

You use the first word of the citation. Usually the author's last name. If no author then the first word in the title.
