I think a good runner up was "Chain Smoking Barbie" the first barbie to chain smoke, a constant stream of smoke poured from her mouth, but if you left the toy on for more than 30 minutes her head burst into flames.
My personal Favorite: The Charles Manson wall graffiti kit, which included switch blades and diagrams to spell PIG and Death to Pigs...etc... on the walls of friends and family.

box of rocks by mattel, sharp pointed stick by ideal, and the ever popular board with a nail in it by whammo. they were great toys enjoyed by millions of kids back in the 60's, but they were considered dangerous by the more repressive parents of the day.

Toy cars were made of steel in the 1950s. My cousin cracked me in the head with one. Also there was a toy stove-oven that burned me too.
