Go to your local Law Library. Most have form books with these in them. In my state (NC) the format for a power of attorney is actually in the statutes themselves.

I believe you can write down and have it stamped by a notary publc (notarized) and it should stand. have a copy for both, also putting it in your will should stand also. I would do both!

put down what your wishes are and Notarize it make sure that you specify a permanent or temporary status
