you can make entries private so that you're the only one who can read them, or lock them so just a few, approved people can read them, or make them open to the general public.

Well you can pretty much put anything you want on www.myspace.com or www.stickam.com, and lots of other websites (tons actually) let you "blog". But a diary kind of implies some privacy. Anything on the internet can be seen by anyone --- is that what you want?

Yes, it's known as a blog and many sites offer blogging. There's blogger (http://blogger.com). Myspace.com has blogging too. I'm not sure about blogger, but myspace even allows you to set your blogs to public (anyone on myspace), friends only (on your friends list) or private (just you).
If you search for "free blog" on google, I'm sure you'll come up with something that will work for you.

I do not know of one. If there was I would avoid it for security reasons. Anything on the web might be hacked and then all your private thoughts might end up being very public.
