You need to use special toughened glass for any table. The glass is a minimum of 5mm, usually @ 8mm but up to 12mm thick. You will have to get a ground edge for safety. You can order it from your local glass supplier already done, but give him the exact size. It takes 4-5 days to come.
The price of glass changes depending on where you live, so look up a supplier in the Yellow Pages. Don't expect it to be cheap, as that's a big piece. (probably @ £80 - £100 ish give or take!) I assume by plain, you mean frosted on one side, not clear!
I got a price a few weeks ago for a mirror 2ft x 3ft in 8mm thick Tempered glass, without safety back and they wanted £127. As mirror is a little more expensive, this will give you an idea of price.

A former glass man here:

What you want is either 1/4 plate or a 3/16 flat glass. Any glass shop can cut it for you. You need to grind the edges, which your glass man can do in a jiffy.

Can't be ordinary plain glass - it must be reasonably thick and toughened. Phone a glass supplier and ask 'cos it'll have to be cut and the edges finished off safely.

It would not be good to use "Plain" glass for that. The safest thing would be a piece of Tempered glass. It will be harder to break and not cut some one.

Any glass shop should be able to cut a replacement for you. Give them a call and ask them..

My guess is $20
