He did make it home for one episode but it was from when he was a boy and he hoped to save his brother from going to war. He was unable to change his family's future, but he realized with Al's help that he should enjoy just being there with his family and perhaps that was all that was intended for that journey.

The original ending has Sam leaping into 1969 a mere minute or two after he and Al leapt out in the episode "M.I.A", to tell Al's first wife, Beth, that Al is coming home. His Vietnam-era picture begins to "leap" (this is where the final episode cuts off), and then we see a modern picture of Al sitting with Beth and their four daughters. This ending somehow made it out of the studio and has been circulated on the Internet. In the ending that was actually broadcast, we are told that Al was reunited with Beth, that they remain married, and that "Dr. Sam Becket [sic] never returned home."
Also, it mentions a show that I missed called "the Leap Back" in season 4 where Sam and Al switch roles after lightning strikes the Waiting Room. This allows Sam to return home to his wife, but when Al's is set to be killed by his lover's jealous ex-fiance, Sam must act as his hologram in order to save Al and continue to leap through time.

Quantum Leap was a great show, it ran from 89-93. There are many different opinions about the ending of the last episode. Though the show was cancelled, there is a web page that has webisodes continuing Sam's leaps from that point on. There are currently 13 seasons and more may be on the way. However the SiFi channel has an article stating there is going to be a 2 hour Quantum Leap movie made. If any one wants to read the SiFi article here's the web address:

Sadly, no, he never made the leap home.
In the show's final episode, Sam spent a great deal of time talking to an individual about the purpose of his travels; and the importance of what he was doing for other people.
Sam talked about "being tired", and wanting a "sabbatical"; but in the end, his desire to continue helping others weighed out over the decision to stop leaping. In fact, in the closing text, is specifically mentioned the fact that he never made the leap home.
