why not get an old pic (from your childhood days) of yourself and your sister (and any other siblings) have it restored and framed (tiffany do a lovely set of frames)
on the back of the pic write a birthday message for her or get the back of the frame engraved with her birthday, it always adds a nice touch.....
this will show that you put some thought into the gift and not gone at got her 'whatever'

Get a little kids bag & fill it with stuff that will remind her of when she was little...maybe a pot of bubbles, sherbert fountain, wax crayons, plasticine, hair bobbles, a French knitting set, marbles...
