It is initiated in the House, which must draft Articles of Impeachment, which must pass out of committee and be voted on by the full House of Representatives. Only a simple majority is required.
For the President, if the Articles are passed, the trial is held in the Senate with the Chief Justice of the Supreme Court presiding. A two-thirds majority is required to convict, at which point the person is removed from office.

There first must be evidence of a "high crime or misdemeanor" by a President, Vice-President, etc.
Since no one has committed such, there's no need for an impeachment.

First they have to break a law and there has to be evidence of it.
Bill Clinton lied under oath on national TV, so the whole world saw him break the law.
The House and the Senate convicts them, the House found Clinton guilty but the Senate didn't even though it was on TV.
