As weird as it may look the last example you gave is correct. I saw it in the obits for a neighbor. Her children were mentioned and their spouses were named as if they were her issue also.

This plaque is presented in memory of Sally A. Jones by her son, Sam and his wife, Betty Jones, I think would be quite appropriate.

To me "Sam (Betty) Jones" is a bit confusing, it makes it look like Sam was once known as Betty (erm.. before the operation..).

The "Sam & Betty Jones" version is better. However, it makes Sam and Betty look like brother and sister.

You might consider something like:

Mother of:
Sam Jones (Son) & Betty Jones (Daughter in law)

or

Mother of:
Sam Jones (Son)
Betty Jones (Daughter in law)

I wouldn't think the daughter in law would be mentioned at all. She's not her mother. The Sam (Betty) Jones makes it looks like that's Sam's middle name and Sam & Betty Jones looks like they were both her kids. No daughter in law.

If you are putting it in the Newspaper as a Memorial then it would be like this : Sam Jones(Betty)

If it is a plaque,sorry only immediate family,sons,daughters.husbands and wife's.

When my great-grandmother died last year, the obituary in the paper read the first way. People are used to reading it that way; don't let anybody make comments about sex-change surgeries.

The first way--Sam (Betty) Jones--is the correct and most common way.

I agree with leaptad and gonicki, a plaque should be for blood relatives only, not in-laws.
