creating Form/s?

Question

I am looking at making a form I can email. I want to have all employes access to this. Basicly I want a tapable form that one can just add information to a pre-designed form without changing the basic makeup of the form.
Any Ideas

HI why dont u use visual basic and try designing forms?
use ms access as back end.
