This is one of those laws that meat manufacturers bought. It states that anything that comes from a cow is classified as beef. Unless you can actually see what the cut of meat is, you truly do not know what you are eating.
If you look online about extruders, you will find that they now use machines where they throw in all the bones after they have been machine trimmed and then the extruder actually squashes the meat off the bones squeezing it through a seive like paste. That's your "patties" at fast food places.
All I can tell you is that cats are carnivores and neither one of my cats will even touch a McDonald's "hamburger". What does that tell you?

Sure- but beef has fat and it really shrinks when cooked out.
SO does it really matter? Do you really want to know "What" part of the beef you are eating? That would upset you more than the 100% part. Trust me.
