What/How should I write my Letter of Absence ?

I am a teacher and I will be writing to the principal of the school where I am working.

Dear Sir:
This letter is to inform you that I will be taking a leave of Absence beginning date and plan to return on date. And maybe extend it by stating your reaso for a leave of absence. Thank him/her kindly.
