If your trying to get out of something, your better off claiming a back injury those are harder to proove.... Jus get really drunk and have someone else take a hammer to your feet, broken toes hurt tho...

I broke my little toe twice, by kicking a chair when I got out of bed in the dark and couldn't see it.

Why do you desire to intentionally injure yourself?

Drop something very heavy on it while wearing flip-flops. You could also have a car run your toes over while you're wearing flip-flops.
