If the slope is undefined, you can't write it in slope/intercept form. An undefined slope means the line is vertical. All vertical lines are of the form y = K, where K is a constant. In the specific example you gave, the line would be y=2.

point slope would be (y-2)=m(x-1),
so slope intercept would be y=m(x-1) + 2
