First of all if your trying to conceive it wouldn't hurt to take a test. To be sure... however I suffer from that myself. anywhere from one - two weeks before my period my breasts get enlarged and are very tender!!! Caffeine I believe can increase these symptoms. I'll research it and put a link at the end of this...
Question
A week before my period I suffer tender breasts but for the past six months they are tender for three weeks out of four and it is really getting to me now.
Can you help?
Answer
As you are aware, it is not uncommon for women to experience discomfort, tenderness and swelling of the breasts during the 7 to 14 days prior to their period.
These symptoms usually settle down when menstruation begins and are thought to be due to fluctuations in the circulating level of hormones such as oestrogen and progesterone.
There has been no shortage of general advice over the years regarding breast tenderness and other symptoms of pre-menstrual syndrome (PMS or PMT).
Dietary changes that have been found to help a proportion of sufferers include – a low fat, high carbohydrate diet, reducing the amount of salt intake during the pre-menstrual time and reducing caffeine and alcohol intake.
Trying to reduce stress, increase aerobic exercise and getting adequate sleep have been advocated and are undoubtedly good lifestyle measures.
There have been numerous medications recommended and tried with varying degrees of success for PMS symptoms, including: vitamin B6, oral contraceptives and non-steroidal anti-inflammatory drugs such as the ibuprofen you are taking presently.
One medication that I have had success with in treating my own patients with breast tenderness is Gamolenic acid provided by Evening primrose oil. This is unfortunately no longer available on prescription in capsule form. It is recommended that three to four capsules should be taken by mouth twice daily, providing a total dose of six to eight capsules. Any response to treatment occurs over a period of eight to 12 weeks due to a gradual onset of action and once this has been achieved, the treatment may be stopped or continued at a maintenance dose.
So, why have your symptoms become worse over the last six months?
Your hormone balance may possibly be starting to change due to a decline in your normal ovarian function and this can be checked by having a blood test.
It is also most important that your breasts are examined to rule out any abnormalities requiring further investigation.
I would suggest that you discuss your symptoms with your doctor so that they can be adequately assessed prior to embarking on any treatment.
Yours sincerely

I will say this -- the ONLY time that I have gotten sore nipples is when I've been pregnant. Usually they start soon after ovulation (usually by 5 days past ovulation -- which for me would be about 7 day b/f my period is due) -- However, i do get sore breasts a day or so before my period about half of the time. I think (personally) that the timing is the key of a good sign or not -- early after ovulation = good sign -- late near period = bad sign. The thing is -- both low progesterone, and high progesterone can cause tender breasts. Mid-lutal phase soreness, it could be a sign of a good ovulation, late lutal phase soreness, it could be a sign of declining progesterone in preparation for your period. Good luck - and i hope it's a sign for you!
