I don't think it costs anything. Go to your local SS office & fill out an application & they send it to you in a few days. You might be able to do it online. They should require ID.

You will need a birth certificate, proof of address. But, you can go on-line and see if the solution to your problem is listed. I don't know if there is a charge for a replacement card but I would guess not.

I would call the Social Security Office and ask them to be sure ,
my guess is that you would need a birth certificate { origanal } .
I just applied for SSI and they had me send them mine .
