By the Nielsen rating as a kid my father brought in this box apparently we were chose for a week to watch TV and to help out with the ratings he attached it to the TV and we had to press this button when we were watching whatever shows we were watching and then after the week he mailed it back to the site and it was cool to see what they do to get the ratings for the TV shows

Some people are given boxes. You don't sign up. The company finds you. These boxes act as a diary that phones in through the cable system to the company's main computer. It then spits out a bunch of numbers and then the company sells these numbers.
These numbers tell advertisers who is watching the program so they can see which hours they want to advertise with. The average age of the viewers that watch Ghost Whisperer is 51 and mostly females. This is not the show that you want to buy airtime for if you are selling motorcycles. Also more of the viewers that you are looking for, the more you will have to pay the networks to air your commercial.
During sweeps, prices for advertisers are set till the next sweeps. Sweeps is when the rating company gets a more accurate system by getting a larger sample. They call people and send forms. This is also when networks stunt by pulling weak shows (permently or temporarly replaced with something that might get higher ratings) and having major arcs or guest stars. You will also notice that in between sweeps, you will see a lot of reruns. Since the advertising pricing is fixed, it doesn't matter if eight million are watching or just 2 people. There are three main sweeps months of November, February and May with one minor one in July. The number of sweeps months have increased over the years at advertisers' request.

Nielsen boxes plugged into the back of your telly.
The Nielsen people pick ordinary folks at random (a sample) and ask you if you want one fitted. Then they count up what those people watched that week and multiply that number by the number of people who have a tv. (Well, basically. I'm sure it's much more complicated than that.)
It's not an exact science and with things like 'sweeps' (when networks put their best shows out when they know the Nielsen boxes are being counted) - which we don't get here in the UK - sometimes the figures are misleading.
Also people who have Nielsen boxes are less likely to watch 'I'm a Celebrity, Get Me Out of Here!' when they think someone is watching them.
