The reason I ask is that I know a lot of us clean up bedwetting and potty training accidents, as well as pet messes, with bleach. If bleach and ammonia are a deadly combination, I wonder if this is really a safe thing to do.

Not trying to be gross...but urine definitely smells like ammonia, if it isn't the exact substance that offgasses with bleach, it must be something very close in chemical composition.

Urine most definitely contains ammonia [so does poop!] I rarely use bleach so I have no idea whether it really does produce the poisonous gas you would theoretically expect. [Though sometimes urine alone can smell pretty deadly!]
For the urine odor on your bed linens and other machine washables, put plain white vinegar into your washing machine's fabric softener dispenser. The vinegar neutralizes the urine, eliminating the smell. [A bonus: no fragrances, VOCs, or allergens.] You may want to set your washer to rinse two times to completely remove the vinegar smell.
Since you are apparently the mom of a young'un, you also might as well get used to adding a scoop of Oxi-Clean to every load.

For your mattresses and carpets, the carpet cleaning guy who also answered your question has it spot-on.

When urine starts to break down the by-product is ammonia. It you left the urine spot alone it would take approximately 7 years for the urine crystals to break down completely.

For cleaning urine Nature’s Miracle works good. It is available at Petsmart. Make sure you follow the direction. The main reason that it fails is that people just sprays it on and wipe it up and not let it soak into the carpet like the urine did. You my need to do this 2 or 3 time. Just reminder that the urine treatment product needs to go where the urine went for it to work properly.
In my experience, Nature's Miracle is your best bet. Good luck!

Just wash the area with soap and anything that is removable throw into the washing machine. If there is a lingering odor use white vinegar and wipe it down good. Use a plastic cover over the mattress to protect it.
