This is so disputed.
The theory I believe is that Leia remembers her real mother, Padme, strongly through the Force. The Force allows her to remember images of her mother she only could have collected through her birth. The shot of the twins in Sith shows the baby Leia looks right at Padme while Luke doesn't. Hence, she remembers Padme and he does not.
The other notion is that Leia is mistaken in the memory of her "real" mother. Who she really remembers is her adoptive mother, Queen Organa, and mistaken believe that is the memory of her real mother.

She has to be talking about Bale's wife, Queen Organa. Otherwise that would be a glaring error in the story. Luke ws raised by Beru, who made no pretence of being his mother. Leia on the other hand was treated as the heir to the thrown of Alderon. She had to be her daughter or else the Empire would know who she really was. So, when Luke asked her that question he could not be certain that they were both disserted at the same time or what. Obi Won, Vader, and Yoda never told him the whole truth. How do you think he would have reactedif Obi Won had told him the whole truth and he learned his father killed his mother? It is more than likely that Luke would have turned to the dark side right then and there. After all, Vader learns about Leia and Luke goes ballistic trying to kill him. And, that was only on the thought his father would do her harm. "Luke, I killed your mother." "DIE!!!!!!" You can see how that could be a problem. Neither of them knew Padme was their mother. There is no telling if they will ever find out. That is why they need to do episodes 7,8, and 9 to clean up the loose ends and bring balance to the story.

I think the real answer is that George Lucas had not decided in 1983 that Luke and Leia's mother would die when they were newborns. As I recall Luke does say 'your real mother', and there is no way Leia would remember her face from the day she was born.
It's like on Star Trek, on the TNG episode with Scotty made in 1992 and set in the 24th century he asked after Capt Kirk, but in Generations, made two years later but set 70 years earlier, Scotty sees Kirk apparently killed.

Luke asks her about her mother - her REAL mother. She answers that she died when she was very young, but she remembers that she was very beautiful. Shes talking about Padme. In Sith the birth of the babies is very pointed. Leia looks at her mother calmly with her eyes wide open. Luke is crying with his eyes shut. Leia is the only one who actually saw her mother.

When she says the line about her mother being beautiful ... I believe she's talking about Queen Breha Organa who adopted her on Alderaan. When asked if she remembers her "real mother", Leia says that she recalls "images" and "feelings" of her. Leia explains, "She died when I was very young ... She was beautiful. Kind ... but sad." She wouldn't remember Queen Amadala at all since she died during childbirth.

Her adoptive mother. Her adoptive father raised her as his own from a young age. Her adoptive mother was killed during the rebellion.

yeah, that bothered me too after Sith, but she does say (in Jedi)her real mother died when she was very young. So maybe she was talking about her adopted mother
