in Ontario Canada, people are entitled to 15 minutes unpaid break for every 4 hours worked.
it's a good idea to take a short break from any activity (especially computer work) every 30-40 minutes for your health

I think it depends on a few factors. First and foremost is the company's policy - remember, you most probably signed an agreement to work under those conditions. Second, the culture of the company - it could be an environment whereby you can take a break when you feel it is necessary as long as you compete your work by the end of the day. Then there is the nature of the job which can dictate when and how often you can take a break. Normally computer data entry and such should allow for a small break every 30 minutes. This break does not mean you can go for a smoke, it means you can do something else like filing. ;)
If you are a surgeon and you are in the middle of a delicate 6 hour operation I do not think it will be acceptable to contemplate taking a 10 minute smoke break.

Where I work its a 10 minute break if you work more than 6 hours. Two 10s & a 30 minute break for an 8 hour shift. But management works 13 hours with no break at all.

In the USA, labor laws are established by each state except when superseded by a contract (like in a union).
In California the state requires a 15 min break per 4 hours work and 1/2 hr lunch per 8 hrs work.
In Arizona, you basically agree to whatever the employers rules are when you hire on, and I often saw crews that worked 8 straight with no lunches or breaks. We had the option of lunches and breaks, but many of us opted for a straight 8.

Every company has a different policy but I think you are supposed to at least get a half hour for every 7 hours worked. I get (2) 15 minute paid breaks to take whenever and 1 hour unpaid lunch. I work 9-10 hours per day.

It depends on your state, you would have to look up the laws. In Minnesota....
Less than 6 hours = 15 minute paid break
6+ hours = Unpaid half hour break and paid 15 min break
8+ hours = Unpaid half hour break and 2 paid 15 min break
I'm not sure of over 10 hours but I think its an hour unpaid and 2 15 paid breaks.
I havent worked anywhere besides MN so I dont know how other states go.

It depends on the state, but most people are shocked to discover that, unless you are a minor, you are entitled by rights to any breaks at all - it's up to the discretion of the company. Since it makes good sense to give workers a break - the quality of work declines without them - most companies schedule two small breaks and a lunch period.

Your suppose to have a 20minute break when your working day is longer than six hours. A rest period of 11 hours between working days. A rest period of 24 hours once every seven days.
