You will have to apply for a replacement card by going to your local office of the Social Security Commission. Have with you your birth certificate and either you state id or driver's license. You will need your originals. You can either apply when you are there or take the form with you to mail in. However, if you mail the form, you also have to mail in your original birth certificate and an id (they will mail you back your documents). I would recommend just doing all that in the office. It usually takes about 2 weeks to get your replacement card.
