It means they have gone all the way sexually with them, i.e they have had intercourse and completed the act.

It's a euphemism for sex, but there is a subtle difference from the baseball "home run" euphemism. With the home run, it's all about the male perspective, he got an opportunity and ran all the way with it, while the woman just sort of stood there helpless and watched. Afterward, he goes to hang out with the guys.
When it's a touchdown, that usually means that the woman threw the pass that started him on his way, and his success was such that he has the opportunity to score again with the "point after".

It would be the equivalent of "hitting a home run," in sports terminology, or "going all the way," colloquially.
In other words, they had sex.
But, you'd have to consider the source; men who brag about that sort of thing loudly in sports bars are probably exaggerating, or outright lying.

In my day ,it meant that the person took the gal to bed. In a Seinfeld show, Elaine tells Keith Hernandez(MVP baseball player) that he won't make it to home, referring to getting her in bed.
