Last name, First name. Title. Publisher's location: Publisher, year of publication.
The title of the book is underlined.

I think this is what you want. I had to call the Library to get this info, as the teacher assigned it after the book was returned to the Library.
Line 1 : Author's name last name, first name
i.e. Anderson, M.T.
Line 2: Title - underlined
i.e. Thirsty
(make sure you underline the title, I can't figure out how to do it here)
Line 3: City and Publisher
i.e. Cambridge; Candlewick Press, 1997
