If you went to your primary care physician, and discussed mental health issues, he might give you a referral to a Psychiatrist of therapist. If you went to a Psychiatrist for medication intervention, he/she might decide psychotherapy was needed instead of or in addition to the meds and give you a referral to a psychotherapist he works in conjunction with. That particular counsellor is not able to help the client for whatever reason. Could be ethical, it could be that the counsellor is not able to connect due to their own issues, may be visting a person centred counsellor and need cognative behaviour therapy. They may need drugs, or sectioned. It just means that whatever the persons presenting or underlying issue, that particular counsellor feels they could be better helped elsewhere!
