ghouls and goblins. not only is it hard to beat, but your guy is quite awkward with some of the weapons.
master blaster. water area. i never got through there.

Ah the good old days. They were all really hard for me because I was about 6. Not like the games of today that you can sit and play through on the hardest difficulty without much time.
I don't think I've ever beaten Super Mario 3 even with game genie. The last levels were killer.
Golgo 13 was tough because I could never figure out what I was supposed to be doing.
The real question I have is if I could get my hands on a good functioning NES system how well could I play the old games, that I really didn't understand. Because I sucked at Captain Skyhawk when I was a kid. I played through it since then when i happened upon a NES. Beat it in one sitting.

The hardest most difficult game I have played on the NES by far has been Shadow Gate I have never been able to pass it. In my opinion Super Mario Bros. would be the easiest game with out cheats or tricks.

OK there was this really crappy game called McKids and playing it more then two seconds will drive you insane as it makes no sense. I never beat it because it was so crappy. I vote for it as hardest to beat simply based on it makes no sense and I doubt anyone would play it long enough to beat it.
