Residential real estate trades for the types of property you describe, given the potential geographical distance between them, are extremely difficult to find and arrange. Best advice is to sell your property outright, and then go look for property to buy in a new location. It sounds like you do not have much real estate experience, so it will be worth your while to rely on the help of an experienced local real estate agent. They can help you sell your property and network with other agents in a new location to help you find a new piece of property. It doesn't cost a cent to talk to them, so if I was in your position, I would talk to friends and neighbors for a recommendation.

Good luck with your real estate endeavors.

I think this is a wonderful idea to try. I know when my family and I moved I told my husband this idea also. We had wished there would be a way of doing it. Of course, we had to move to Johnson City, Tn. (or surrounding area) due to my father-in-laws health problems. Too bad there is no business like this, it could help a lot of people. We had to down size, but needed a larger home. We had to move up from Florida, but there just wasn't anything in our price range available. I will pray for you that something comes along! Good luck!!

Be prepared for a lot of work. You could tried listing it on craigslist.com or maybe even e-Bay. But what I would do is decide where I want to live then contact realtors in that area. Many realtors have sellers looking for just the right deal.
I wouldn't be very optimistic. Maybe you should just sell your property and rent in the area you like. You may find it's to warm or humid in summer and you'll still have your options open.

You need to sell your property first. Move to an area you think you like and with healthcare you have checked out. Live there for six months and then look to buy.

Can you find someone who will 'trade' you? Sure, but remember you will be getting the short end of the stick.

What a great idea! Have you put in add in free classifieds on the internet?
Where are you located? do you want to move to a warmer climate?? etc. Put all your information in classifieds, for a trade. You might be surprised at the responses you get.
