If a river is expected to crest at, say 13 feet, from where does that measurement begin? River bottom? average river surface?

During conditions when the river is rising and flooding is a concern, the elevation at which the river "crests" is the maximum elevation it will achieve, as was already stated by CanTexan.

During some times of the year, a river's average level will rise or fall depending on certain circumstances (tidal action, heavy rainfall, snow melt, damming, exteneded periods of drought, etc).

"Cresting" refers to the times when the level rises; it is actually the highest level the river will achieve.

Cresting is when the river is high enough to makes waves or to rise to the top.
