A card or ecard is good enough.

If you give a gift to a co-worker, it does not look good if you do not give one to others, and you may not want to give every year, don't you?

It depends on how close you work together. Is this person in the same area as you or do they work across the room? Do you have contact with them on a daily bases? No matter, I would think a "happy birthday" would suffice, as that would be all you would expect from them for your birthday.

if you are talking about like a christmas gift.. yes.. a card and maybe a little christmas decoration or homemade goody.. or if not that put a gift card in the card for a coffee place for a coffee or two

No...why would you? Just sign the card somebody will pass around. (You wouldn't expect one from her/him, would you?)
