I had to do a report on the poem the Wasteland by TS Eliot when I was in school. I don't remember much about it, but my assigned writing partner tried to kill herself in the middle of the project...don't know if it was the poem. Probably not, but still...TS Eliot is not exactly a happy writer.

The most depressing poems that I ever read was about the stories of a poet in Iran, said from his childhood stories to today! The Iranian very good singer "Googoosh", and with a very good muisc on poems made me very very sad; AS I found she was singing 'My Life Story' how we played at childhood, our childish fights, games and LOVE & KISS away from parents, and then, at high school and college, all those hot political argument that Marx is beter then everone less; and then the CIA conspiracy against SHAH and death of ten thousdands innocent people,,,,

I've read several depressing poems, mostly because they were so badly written. The form you're looking for is Melancholia. Poe has several.

For poems, try Thomas Hardy. He's pretty bleak in some of his poetry. But the lyrics to "Left and Leaving" by a band called the Weakerthans has stayed in its position as the number one saddest song/poem I have ever heard in my entire life.

"Daddy" by Sylvia Plath. The last line, "Daddy, you bastard, I'm through," has a resounding despair. Plath committed suicide. The poem is unbearably bleak.

"Lady Lazarus" by Sylvia Plath, a poem by that great poet about another of her failed suicide attempts. (She was finally successful in it later.)
