It's exactly the same as a personal assistant, or secretary, who specializes in medical terminology. Mainly working with audio transcription equipment like a voice recorder, they will listen to the recording and produce the document from that recording.
It requires essential specialized training, as a lot of medical terms are really unbelievably difficult to spell, and 100% accuracy is required.

i think some doctors record notes to themselves as they are doing a particular procedure. they will have someone type out the transcription. no?
