I think many cultures, particularly Asian cultures, place much more emphasis on appearance and thinness than American culture does. The pressure and response differ by group. In America, I think we are far less likely to have cultural values emphasizing discipline and exercise and less healthy understanding of eating growing up (i.e. McDonalds, cheaper is better, more is more). Our response to obesity or body dysmorphia may be equally extreme--binge/purge or starve.

Additionally, mental health concerns are are more taboo in eastern cultures--resulting in underreported and decreased incidence of treatment of mental disorder. Therefore, there isn't much literature to fully document and characterize each culture's incidence of eating disorders. That doesn't mean it doesn't exist.

In my anecdotal experience, my thin Asian friends get very blunt criticism about their weight when they visit their home countries. Family members will call them fat. Clothing shop employees will turn them away saying they are too fat and they have no clothing that will fit them. Perhaps if Americans grew up in that type of a culture, we would have less obesity to begin with.
