I've always understood it to mean an extremely small amount. As in "I'll just have a titch of cream in my coffee." There may be other meanings I'm not aware of, but that's how the people around me use that word.

Little Tich was the stage name of Harry Relph, a music -hall comedian (obit 1928). He was 4 foot 5 inches tall and performed a dance routine in enormous boots. He was called 'Little Tich' as an ironic reference to the fat person who claimed to be Sir Roger Tichborne, a missing heir.
