Get the company requesting the reference to chase this up... quite often a phone call from them is enough...
If your previous employer has given references to others then they can't refuse to give a reference for you, as this would be a discriminatory action.
Be aware though that employers can give a bad reference provided that what they state is the absolute truth (or the truth as they see it - which is dodgy).
Some employers can be quite vexatious as they see an employee leaving as an act of treachery which they feel inclined to punish. Advise that you have tried to get your ex-employer to co-operate and that you suspect that they are being difficult and then eave it with the HR people at your future.

No employer is under any obligation to give you a reference, good or bad.

You can't force them to give you a reference. If you're on good terms with your ex employer and honestly think they have lost the form you can ask your potential employer to email them a copy. Otherwise see it as your ex employer being tactful...

If your employer is that nasty and awkward, find a senior member of your company who will give you a private reference and explain to your prospective employer that there is a conflict between you and your current employer about your leaving. That usually works.

umm... you get a little extra section to describe the question.

Employers are under no obligation to provide a reference. If they're claiming they haven't received it, they're just being petty. Scare them by saying it's against the law though. Might work.

How come you didn't finish the question? I would just explain to the people wanting the reference the trouble you're having or find someone else to do it for you, like maybe a colleague or someone.

If you can absolutely prove they are blocking a reference they may have a problem. But you had better make sure you receive a copy of what was said.
