If I were his "shrink" we would explore issues dealing with loss and balance.
Loss of his first wife and loss here with this one. Also how the tabloids are making matters worse and how to carry himself as grace under pressure. also his children, all of them.

I believe his main concerns are his children. He has handled celebrity as well as one can and is used to it.
Most people when aging are not upset about it but do have concerns for their health so that too would be something to explore in counseling. I'm sure he already has people close to him to fill the role you suggest and I doubt he would come to me for advice anytime soon.
He has dealt with loss before and this is just one more thing to deal with. The loss of Linda and of John and George have made him stronger so he may not need any "shrink" assistance.

Paul's real problems were his eyeballs.

He made the same mistake of marrying a shrew that John Kennedy Junior made 5 years earlier.

John married a very pretty lady who turned into a shrew once she had her fangs into John's wealth via marriage. The only reason they did not file for divorce was because they did not live long enough for her to think that she had yet earned her big payday with enough years of being married to him.

Paul made the same mistake with his wife. He married someone who saw a huge payday after four years of marriage. She just hid it from him during the engagement.

Donald Trump did it right with Marla Maples 15 years ago. He had an absolute ironclad prenuptual.

The football player Troy Atmen (sp) from the Dallas Cowboys ten years ago once said that he can meet many women, but he was not yet knowledgeable enough to see which ones were being nice to him because they liked him and which ones very being nice to him because they liked his money. That's how he explained why he is not yet married. The football player is pretty smart.

There is an old song from 30 years ago, "If you want to be happy for the rest of your life, NEVER make a pretty woman your wife. ... get an ugly girl to marry you."

Myself, I'm very happy.
