Have the birthday celebration early in the day so he does not get all stressed out waiting...like a birthday breakfast with cake and ice cream or birthday breakfast then when guest start arriving, spend time to do the song and cake and ice cream kinda like the appetizer course...on Thanksgiving Day a little cake and ice cream just this one time will not spoil anyone's big meal. And make it have a birthday theme for the breakfast or appetizer celebration...try not to double up the turkey day theme for the birthday and holiday.

Make him a special birthday breakfast.
Let this years dessert be his birthday cake. Decorated just for his birthday.
If any other children are coming for Thanksgiving, decorate the childrens table for his birthday "party".
Make sure everyone wishes him a Happy Birthday

You could get a cake from your local grocery store that has a thanksgiving theme but also put happy birthday on it and put up streamers that are also thanksgiving like colors and have 1 item at dinner that is his favorite just for him.

Do a little something to recognize it on Thanksgiving but give him his own special party over the weekend, with just your immediate family.

At least it's not every year like a Christmas B-Day!!!

Turn Thanksgiving into a birthday party. Tell everyone what your plans are so they can buy him something.

All you need is some balloons up and make people wear hats during the dinner. Tell them it's his birthday so they bring gifts for him especially the other kids. And instead of pies have a big birthday cake at the end of dinner.

The best advice I can give on this is:

Dont forget he is there and remind him that is its his birthday(theres the birthday boy)etc

And maybe do something special for him that weekend.
