I've seen a few mention movies such as The Usual Suspects and The Sixth Sence (both are great movies), but I'd like to include two more- The Sting and Shade. Both movies deal with con-artist and have twist and turns throughout. The end of The Sting was smooth and went right in line with the entire con job. Shade left me with a grin thinking how clever it turned out.

I'm shocked that in 37 answers nobody has mentioned memento, the twist in this movie is just brilliant and yet the way the director shows us the twist is just the perfect recipe, the fact that the twist is at the start of the movie, and the way the movie plays, we only get to see at the end, that is what good movies are all about

Have you seen the movie 'Gossip' with Kate Hudson? That has a brilliant twist, but I won't spoil it. It is one of those movies where you are totally shocked at the end. I have seen it twice, and the second time I watched it I had forgotton the twist and ended up being surprised all over again!!

Saw. The final scene in the underground toilet. Where the dead body isn't actually dead and has been laying there since the start of the film whilst everybody else is either dying or mutilating themselves. And it turns out that he is "Jigsaw" who was carrying out all the killings.
A very bizarre end, not for the faint-hearted and well worth a look if you haven't seen it
But don't forget "Twister"

In Primal Fear where an altar boy is on trial for the murder of a priest. It turns out he has split personality syndrome and he gets off because of this. Then at the very end its revealed he doesn't. I know i'm not explaining it well but the way it's done in the movie is very shocking.

I've narrowed it down to 3: Wait Until Dark (when the robber jumps out on Audrey Hepburn), Don't Look Now (when you realise that Donald Sutherland had foreseen his own funeral) and Fatal Attraction when you suddenly realise Glenn Close is NOT dead!
Sorry if I've spoilt the surprises in these 3 films!

I think either in Fight Club when you found out the Edward Norton is schizophrenic and that Brad Pitt only exists in his mind and that hes created a mad cult of people intent on blowing up the world.
Or maybe in the Usual Suspects when you realise that everything you heard in the film was totally wrong and it was all a cover up for the main character to escape

I've got to say my vote goes to the first Saw. The bad guy was lieing apparently dead by a gunshot to the head in a pool of congealing blood in the middle of the room, all through the film, and at the end he suddenly gets up and walks out (dont want to spoil it too much for those who havent seen it, so I shall say no more).
Woah!
Or, Edward Norton's character in the custody cell with Richard Gere in Primal Fear. Brilliant.

The above answers are good. But if you're looking for a movie so full of twists that you will not know what happens until the last second of the film check out the film "Heist."
