Go online to Mrs. Fields and other cookie sellers, look at their order forms, do screen shots, and replicate in MS-Word - you can insert tables in Word to do the ordering, easier than doing the text in Excel.

Office supply places ( Staples, Office Max) carry sales books if you're in a pinch. You can also make forms in computer programs like MS Word, Excel, or Appleworks if you're a Mac person. Hope that helps!

You can use a Form within Microsoft Word. Show your Form toolbar to see what options you have there.
If you're not technical, you can always go buy pre-printed generic form tablets.

if you go to the staples dept store they sell a order book , in packs of three, it make a copy so you know how much one person ordered and when
