tamaso ma jyotirgamaya
mrtyorma amrtam gamaya

Meaning:
Lead me from the asat to the sat.
Lead me from darkness to light.
Lead me from death to immortality.

Its "Brihadaranyaka Upanishad"

The complete thing is an upanishad, the individual lines are mantras.
