You can have a glass shop make you a new glass table top, but I am guessing that the custom made top would cost more than the whole table. I would go ahead and get a price, but you may well find that it is cheaper to buy a whole new table.
Good luck with your table.

Make a template out of paper (news paper taped together works well -- just trace around the frame of the table) and take the template to a glass store -- most windshield repair/window repair places will but to your specifications.

Take a measurement of the table frame & go to a glass shop, they'll cut it for you.

if your table is round then take the measurement of the radius. measure from the dead centre to the out side edge and go to a glaziers or a diy store that stocks glass and they will cut it to size and polish the edges for you.

Measure what size you need, then go to a Lowe's or Home Depot. They will custom cut it for you.
