The largest death toll in a school related violent incident was 39 in what was known as the Bath School Disaster back in 1927, involving several bombings by Andrew Kehoe at Bath Consolidated School in Bath Township, Michigan.

The largest involving student violence was the Columbine Massacre in 1990, instigated by Eric Harris and Dylan Klebold in Colorado that left 13 students and one teacher dead (including the shooters themselves).

Wrong. The most was New London, Texas. A natural gas leak exploded and killed, I think, over 200 people. It was in the 1940's. A convent moved to the area and set up a hospital for the wounded. It is now one of the largest hospitals in East Texas.
