I have friends in Poland, I am in America. Now the Temperature around 0 C at night, so, they are having some colder days. Though some days may get up around 5-8 C warmer. So, take some warmer clothes and a few cooler clothes for sitting around indoors, I would guess.

It will probably be somewhere around 8 -10 C and rainy. So clotheswise, usual stuff really, coat, scarf, boots.
Yes. You need a passport or EU ID card because you are crossing an international border. If you are going by train, be careful as there is occasional trouble on trains from Germany to Poland, especially round Gdansk.
