Your girlfriend needs to go see her gynecologist for a repair. She may have been sexually abused when she was younger or have some type of fistula which can cause even a bigger problem.
I suspect she has not been to a gynecologist, so now may be the time. Please handle this with gentleness and dont just blurt out something about sex abuse when you talk to her about it, but be kind and supportive. Offer to go to the gynecologist with her.
I am a nurse, and her problem can be surgically repaired. It is done as an outpatient or an overnite stay in the hospital.

some women are just naturally small. make sure you are always careful when first penetrating, use plenty of lube, and only start when she is comfortable. some positioning may worsen the tear, so experiment with different positions. if this continues to be a problem see a gynaecologist, they may be able to help her

The tear is probably from inserting bowling pins and the like inside of her vagina. Dumb trick....

Your girlfriend needs to go see her gynecologist for a repair. If she doesnt it could just get worse or also infected.
