I need the format of salary certificate to be submitted for Income tax purposes, if anyone has one, please share it with me. thx.

Tax office should have their forms that you fill out and sign.

In USA I-99 is used for this purpose.

Are you certifying for your employee?

Is your employer certifying for you saying they have paid it.

Go to office supply stores they will have general forms.

They may call them legal documents.

Are you yourself certifying that your salary was so much?

In any case you need to get the correct form, from correct authority and have it filled and sign by correct authority.
